Summary Of A Student Nurse: A Case Study
A situation that I observed, which did not meet the five rights of delegation, was a student nurse starting an IV on a patient. The registered nurse (RN) caring for this patient was not successful gaining IV access. She knew the student not only had experience with IVs, but worked on a unit where nursing assistance could start IVs. Once the RN was unsuccessful, the RN allows the student to start the IV nevertheless she was successful. Although this may be an appropriate task for a nursing assistant it was still not appropriate for a nursing student. Considering she was there as a nursing student starting an IV was out of her scope of practice. That’s why I feel this situation was not meeting the right task of the five rights of delegation. …show more content…
The student said, “Let me try, I am normally the one who does this at my job anyways”. The RN should have clarified that students are permitted to start IVs with the clinical instructor, if she was unsure. If the RN knew students are not qualified to perform this task then she should communicated that to the student. If the student was unable to understand the purpose of this guideline then the RN should educate the student. To assure both parties understand what is in their scope of practice, expectations should always be clearly stated which includes limitations (Marquis& Huston, 2017). If boundaries and expectations are set during the beginning of the RN/student nurse relation then clarification would less likely be needed during the shift. However the clinical instructor should be involved in this situation because this is her student. The RN has responsibility for her patients, and the clinical instructor has responsibility for her group of students. Since the student was allowed to complete this task once she may feel it is acceptable to continue to do. If the instructor was notify before the task took place she could help reinforce the limitations to the student with the

    Miss R was married but separated from her husband. She had two young children that were in the care of her parents due to the husband’s abusive nature. Miss R was suffering from Renal Cancer and at the end of her life. She was still able to speak for herself, but pain was becoming an extreme problem. She decided to put herself on our “Comfort Care” orders, which are the equivalent to Hospice. At this time her husband showed up and started wanting to become part of her care. This scared and worried her terribly. Immediately I contacted the charge nurse and the Ethics Committee was notified. This committee consists of social workers, BSN nurses and hospital pastors. They were able to address the situation, the law, the patient’s current needs and quickly develop a plan. A DPOA was drawn up and signed while patient was still conscious and of sound mind. She made all her wishes made very clearly and succinctly, and the decisions for her care were then legally transferred from her husband to her mother. I am sad to say that Miss R passed away less than one week later, but am glad to be part of a team that was able to pull its resources and do an amazing job. I think earning my BSN will make me a more valuable part of that…

    After gathering our patient’s information we got a report from the previous shift. The nice part was that my preceptor had most of the patient we had the day before. So she was well informed about the care of these patients and was able to give me detailed information about each of them. It makes the work easier when the nurse knows her patient. This week I started my clinical by helping a wound nurse who came to assess and change wound dressing of our patient. That patient was admitted for overdose and with some infected wound on the back of her both legs.     My first instinct was just to let it go, and not do anything.
