In substance abuse the care plan is consider to be the treatment plan, this treatment plan address two individual concern. The plan consist of three objectives to each concern, which will be review after 60 days. The only concerns toward developing a treatment plan involve the practitioner lacking the ability and skills set require to assist the individual with achieving success with meeting the objectives of the treatment plan. The Wedge participation toward arranging services for the individual is extremely limited. This is primary due with the Wedge only providing individual and group sessions. The Wedge in content with this particular approach toward providing services. Thinking outside of the Wedge approach toward delivering services will produce a therapeutic environment that will enhances the individual holistically with attaining
