12/08/15 pre-authorization letter indicates that this is an appeal of the denial for participation in the Active Rehabilitation Program. Stemming from consumers’ increased demands for enhanced quality of care and greater accountability from health care organizations, leaders must engage in decision-making that incorporates evidence from well-conducted research whenever possible. The Informed Decisions Toolbox (IDT), a process of evidence-based decision-making, involves six steps in the decision-making process (Rundall, et. al, 2007). While every step is consequential, step two, the process of gathering evidenced-based sources of information, is the most important tool in decision-making. The IDT plays an important role…

We are constantly being flooded with information and everyday this progresses more. Learning to perform excise clinical judgment requires extensive, life-long study. iii. The third challenge that is faced by the Partners Institutional management is the need for knowledge management processes to derive the best practices from the observable outcomes of recommended medical therapies (Joseph Tan & Payton, 2010). As the healthcare industry is making rapid advance, many health organizations are faced with new methods and procedures in regards to quality care. Evidence-based practice can be defined as decisions that are made based off of previous knowledge and research. The combination of clinical expertise and evidence gained through clinical research results in the method of evidence-based practice (Atkins, Fink & Slutsky 2005).…
These restrictions are a concern within the physical therapy profession. Common limitations are number of visits, time per visit, restrictions on patient populations that have direct access, and requirements of previous diagnoses. Treatments available without physician referral are currently limited and physical referral is still mandated for certain treatments (Direct Access at the State Level, 2015). However, Montana law states there are no such restrictions and a licensed physical therapist can evaluate and treat an individual as long as they do not go beyond their scope of practice and limitation of training and education (A Summary of Direct Access Language,…
Quality decision making is probably the most important component of good clinical practice. If we are to appreciate, analyze and improve clinical decision making, it is important that, in addition to understanding the features of the immediate clinical problem, we state the contextual factors that are taken into account when making decisions. Nowadays evidenced based practice is used to improve quality of clinical practice. If the FNP is to promote effective decision making, she will have to have the knowledge and experience to focus on the entire problem at hand rather than the immediate problem.…

A documentation style is a standard approach to the citation of sources that the author of a paper has consulted, abstracted, or quoted from. Documentation prescribes methods for citing references within the text, providing a list of works cited at the end of the paper, and even formatting headings and margins. Every academic schools uses different documentation styles. Documentation styles allow you to give credit for secondary sources you have used in writing a paper. Citing sources also allows your readers to locate the sources you have consulted. Documentation style is important because it allows you to give credit where credit is due.…
The interactions and decisions made when caring for Mr. Smith were largely guided by the CRNBC Professional Standards, especially Standard 3: Knowledge-Based Practice (CRNBC, 2012). When collecting data on my patient I used variety of sources before using the data to guide my diagnostic process and interventions (CRNBC, 2012). This includes, the physical assessment of the patient, looking up the patient’s history and communicating with the patient (CRNBC, 2012). For example, the appearance of Mr. Smith’s legs looked healthy with strong bilateral push and pull strength but when looking at Mr. Smith his legs looked unproportioned to the rest of his body. Upon communication with him I discovered that he had been bedfast for most of his hospital stay and had lost much of the muscle mass in his legs. This helped reinforce the idea of using multiple sources of information to assess a patient because everyone is different, normal for some may not be for others and the best…
Independence is a capability that no one wants to lose for any amount of time; which is where therapy comes into the picture. There are many different types of therapy, two of which are occupational therapy and physical therapy. These two branches of rehabilitation are very similar and often confused, but they are not the same. Physical and occupational therapies often work together and overlap on their treatments. Even though they intersect the therapists are usually looking for a different outcome. The difference in outcome is what makes these two types of therapy crucial to each other. Contrasting occupational and physical therapy will show more clearly why both are needed in their own right; these two therapy categories complement each…
