1-Antonio is definitely feeling stressed. He has too many tasks to perform in a short amount of time. Also he has become the team lead just recently. Anytime one begins a new position there is some amount of stress because of the unknown. New nurses usually have the skills required to perform their job, but can lack management skills, like time management, to get the job done. Management skills are acquired through experience and time.

2-Anotonio could have a to-do-list that might help. If I had to design the list it might look like this: a) Receive report on assigned patients. b) Decide what order the patients will be seen. c) Delegate tasks to others. d) Place unique tasks on the list that need done for that day. For Antonio, that means schedule when to meet with team members so that they can be evaluated. Also as the team leader, Antonio needs to review admission and discharges for acuity and document reasons for exceeded length of stay. This should be done daily so that at the end of the week he can tally up the results and be able to prepare his monthly report.

3- In order to analyze his activities, Antonio should have a time log. Lets just say for this log he works 7a-7p. So at 7-Receive report. 7:30-Prioritize patients' tasks and delegate. 8:00-Gather any supplies and equipment needed including checking the patients' medication administration record to order any meds if needed. 8:30-Begin patient care including med administration. 9:00- etc............The important thing to remember is to continue to record every 30 minutes. Schedule meetings and time to meet with team members. Antonio should continue to have the to-do-list throughout the whole shift. He can evaluate the list to see what needs to be adjusted in order to help with time management skills.

4-Antonio can organize and streamline his work. First, he needs to look at the situation and anticipate any crisis that may occur and be prepared. Gather supplies/equipment needed for each patient,

    After observation of handling the care of Ms.R by Ms.W the need for a plan to promote interdisciplinary care/communication and the maximum utilization of it, would definitely be an important process that requires intervention. One of the steps that I, as the new nurse supervisor would implement, will be to set up a daily stand up meeting where all the disciplines are involved for at least 30-60 minutes where we discuss and go over all the patients and make notes on what the pane for their treatments are. Given the situation, an end of the day meeting would probably be more efficient where all the disciplines can address the issues that they had during the previous day and discuss into determining goals that would be available to work towards them by the beginning of the next day or with the new shift staff. This will be an almost immediate intervention and would start addressing the issues at the earliest most convenient time. During the meeting, it will be mandatory where the discipline in charge of their patient(s) will have to disclose the steps taken so far, goals to be achieved, interventions planned and the time frame they strive to and/or plan to achieve these goals. Since all the disciplines will be present in the meeting, it will be a good tool to utilize an opinion sharing and collegiality experience to help in suggesting, advising and offering an intervention that might be beneficial and result in positive outcomes for the patient(s) that will be discussed on.     While I was not involved with all of the four functions of management, I did see the different functions being implemented throughout the office by the therapists, the receptionists and the trainers. As far as planning goes, there was a dry erase board used with the lists of names of patients being seen that day. The trainer wrote down many different exercises to be performed that day. They did this to help the process go more smoothly. There are far more patients than therapists and trainers so this way the patients could walk in, look at the board and know where to begin after heating their injuries.     The main problem that occurred during this clinical was time management, constantly checking for new orders, and knowing the time schedule of each of the patient’s medications and keeping track of their next medicine. To help with organization, I kept a time frame of the various times we had to give medicines to the patients on my report sheet. For instance, time management was difficult during the first day because charting each morning assessment took a long time and there were always interruptions or new activities that postponed charting. Overall, further improvements include becoming more confident in communicating with physicians over the telephone and in person to ensure patient safety. As I gain more experience, this will improve.
