9:00-9:47AM. SC received an email from OLTL noting approval for increase in PAS hours. SC made required changes in SAMS and Oracle. SC contacted the provider Home Health Care on 12/31/2015 at 9:19 and spoke with Ferline an after hour’s receptionist. SC left detail message stating reason for calling and contact information. Ferline states that the office will be opened at 9:30 and she forward SC message. SC thanks Ferline and end call. SC place call to Pa caregiver Caroline and informed her of service approval. Caroline was pleased to hear stated that she Pa’s current DCW ( Ruddy) is no longer available to provide care to Pa in the same manner he was able before and she is in search for a replacement. SC suggest Caroline use an agency aide until she finds a suitable replacement because Pa’s needs are high and bot having adequate help can leave her burned out and Pa at risk greater declined. …show more content…
Caroline reported that Pa is able to set up but unable to stand or walk. Caroline reported that it’s very hard on her to get Pa in/out of the house in his wheel chair if Ruddy or the neighbors are not around she can’t take Pa to his medical appointments. SC informed Carline that a referral for a stair lift assessment has been done but in the main while SC with submit an attendant transportation service application for Pa to have the proper help to ensure safety getting in and out of the home so that Pa continues to received medical support and treatment uninterrupted. SC informed SCS about the matter

SC made an ordered OT evaluation from DME provider All Care Medical and several others SC was unsuccessful in obtaining an appt because the DME provider does not have OT or PT on staff.
