Yommala had called the Supervisor for assistance as the patient was emotional during her counseling session. The patient wanted to seek medical attention at the ER and reports of no SI/HI. According to the patient and Yommala, this patient is currently in a DC situation with her ex-boyfriend, whom continue to harass the patient the clinic’s premise. The patient was advise that the clinic would contact 911 if the harassment should continue and for the patient to also contact the police department in her local town as well. During the session, the patient changed her mind about going to the ER but wanted to see mental health provider immediately rather than waiting about two weeks. It was recommended for Yommala to assist the patient with seeking a sooner appointment, like today to see her therapist if applicable. After this encounter, Yommala updated the Senior Counselor about other problematic issues with the patient, referring to her DCF case and stresses in her personal life. Yommala briefly discuss her plan to help her patient with her current dilemma.…

Lynn had a unrestful night and into the morning of 7/15/15. The morning medication was adminstered at 7:00 pm. Although Lynn had been given her morning medication and had eaten breakfast, she began requesting and demanding her 1:00 pm medication. At approximately 8:00 am, Lynn’s behavior escalated and she began to attack the support staff, DSP Clark and DSP Seay. Lynn pinched, scratched, chased, and threatened harm to both DSP’s. Lynn would demand medication and then have moments of sobbing cries, in which she would beg for an overnight stay at a hospital. A phone call was placed to Mobile Crisis at approximately 8:30 am. UCM: CPSW called Ms. Sadia to schedule a home visit and to discuss about her UA's and the updated therapy she had mentioned during the last home visit. Ms. Sadia stated that she has called the therapy and still waiting a phone call from her therapy to schedule an appointment. I thought your idea to directly ask the caller about their chief complaint was an excellent one. In my experience as a transplant case manager, I found members would express their complaints clearly if a rapport of trust was established early in the call. However, you are correct in that our “elevator pitch” did not question chief complaints. Instead, every value driver was to be address in sixty minutes or less if metrics were to be reached. In my elementary school, I would include nutrition and health services, integrated case management, and programs to help link the parents and community to the students. We all want students to learn and progress in every area possible; many times, student achievement and growth is hindered by factors before occurring before the students even get to school. When students go to school hungry or not feeling well, learning is most likely not the first thing on their minds. Also, I believe integrated case management is needed because often times, students and families who qualify for and need one service qualify for and need other services as well. These services are often offered by separate agencies, and case management would help to bring some congruence to the students and families and human services delivery. (Smith, Jr., 1995) Teachers often remark that parent involvement is lacking in schools. One reason found is that schools operate during work hours, and many parents are unable to leave work for appointments, especially those for whom every penny counts.
