AirCrash During Approach to Landing, Empire Airlines Flight 8284
Avions de Transport Regional Aerospatiale Alenia ATR 42-320, N902FX
Lubbock, Texas
January 27, 2009
Executive Summary
Aerospatiale Alenia ATR 42-320, N902FX, operating as Empire Airlines flight 8284, was on an instrument approach when it crashed short of the runway at Lubbock Preston Smith International Airport, Lubbock, Texas. The captain sustained serious injuries, and the first officer sustained minor injuries. The airplane was substantially damaged. The airplane was registered to FedEx Corporation and operated by Empire Airlines, Inc., as a 14 Code of Federal Regulations Part 121 supplemental cargo flight. The flight departed from Fort Worth Alliance Airport, Fort Worth, Texas, about 0313. Instrument meteorological conditions prevailed, and an instrument flight rules flight plan was filed.
Probable Cause
The National Transportation Safety Board determines that the probable cause of this accident was the flight crew's failure to monitor and maintain a minimum safe airspeed while executing an instrument approach in icing conditions, which resulted in an aerodynamic stall at low altitude. Contributing to the accident were 1) the flight crew's failure to follow published standard operating procedures in response to a flap anomaly, 2) the captain's decision to continue with the unstabilized approach, 3) the flight crew's poor crew resource management, and 4) fatigue due to the time of day in which the accident occurred and a cumulative sleep debt, which likely impaired the captain's performance.
Recommendations
As a result of this investigation, the National Transportation Safety Board makes the following recommendations to the Federal Aviation Administration:
Require that role-playing or simulator-based exercises that teach first officers to assertively voice their concerns and that teach captains to develop a leadership style that supports first officer assertiveness be included as part of

For Bangladesh, the readymade garment export industry has been the proverbial goose that lays the golden eggs for over fifteen years now. The sector now dominates the modern economy in export earnings, secondary impact and employment generated. The events in 1998 serve to highlight the vulnerability of this industry to both internal and external shocks on the demand and supply side. Given the dominance of the sector in the overall modern economy of Bangladesh, this vulnerability should be a matter of some concern to the policymakers in Bangladesh. Although in gross terms the sector’s contributions to the country’s export earnings is around 74 percent, in net terms the share would be much less partially because the backward linkages in textile have been slow to develop. The dependence on a single sector, no matter how resilient or sturdy that sector is, is a matter of policy concern. We believe the policymakers in Bangladesh should work to reduce this dependence by moving quickly to develop the other export industries using the lessons learned from the success of apparel exports. Support for the apparel sector should not be reduced. In fact, another way to reduce the vulnerability is to diversify the product and the market mix. It is heartening to observe that the knit products are rapidly gaining share in overall garment exports as these products are sold in quota-free markets and reflect the strength of Bangladeshi producers in the fully competitive global apparel markets…

Meanwhile, TC also introduced initiatives and an emergency directive, and even the U.S. Government issued procedures for railway safety. When the TSB finished its investigations 18 distinct causes were identified from which 16 were taken as a risk ranging from the danger of leaving trains unattended to not having emergency response plans. In January 2014, the TSB released three recommendations aimed to the railway companies, and in August 2014, TSB released two more recommendations, this time aimed to gubernamental institutions. Finally, the TSB concluded that the accident was not caused by any individual but by the combination of factors that the TSB will be continuously monitoring until the measures have been adopted properly.…

At Los Angeles International Airport, a horrible accident by a continental DC-10 took place On March 1, 1978. According to some continental airlines experts the reason behind the accident was “two tires burst on the left landing gear”1. Despite the efforts of the captain to stop the plane, it was too late to control a plan that ran off the ramp with at 85 miles per hour. As a result “The landing gear broke through the tarmac, burrowed into the ground, and was ripped from the wing, making a 3.7 foot hole which allowed fuel to pour from the wing fuel tanks. The plane was severely damaged by the resulting fire and rendered unrepeatable”2.…

Among many contributing factors to the crash of Flight 1572, the most significant accident factors were environmental hazards. Due to severe weather conditions in the North Eastern U.S., there were many weather related contributing accident factors to the Flight 1572 crash. Firstly, according to the initial report that the flight crew received via the Automatic Communication and Recording System (ACARS), conditions like severe turbulence, icing at lower altitudes, high winds, reduced visibility, rain, and low-level wind shear were expected on the approach to BDL. The reason behind these extreme weather conditions was the rapid change in pressure around the Bradley area. Even though the flight crew received several Significant Meteorological Information (SIGMET), due to extreme conditions, the information they got was outdated in short notice.…

The areas researched are (1) obtaining a management degree, (2) becoming a licensed pilot, (3) government regulations, (4) aviation finances and (5) aircraft maintenance. The suggested type of degree would be bachelors in airport management and/or professional aerodynamics as well as participation in internships and airport executive professional organizations. A pilot’s license is not only recommended but highly suggested. The manager needs to be very knowledgeable and well versed in the area of government regulations such as the FAA and NSTB. Aviation finances are a special area of focus because of the importance of running a fiscally responsible organization. Finally, aircraft maintenance understanding is the main area that will be detrimental to the success of the aeronautical prevalence.…

During the preflight and the flight the crew demonstrated how the lack of communication of small details can be deadly. The flight crew should have discussed and medication they were taking during the preflight. Also the flight crew should have discussed the decent and the landing while performing the steps. While in a holding pattern at Martinsville/Blue Ridge Airport the flight crew was given a minimum decent altitude by the controller and during their decent they should have notice they were well above that altitude. Had the flight crew been discussing the instructions given by the air traffic controller and their actual landing path they could have noticed they were too high to land and avoided the accident. In turn is the flight crew had done the same with the instructions of the flight path that was given by the air traffic controller if the landing had been missed they could have notice they were off path. Since they did not communicate the flight plan they did not realize they were off course from the directions given by the air traffic controller. When you look at the flight path they should have fallowed if they missed the approach you see they would have clear to come around and make another…

While climbing to the new altitude the airspeed dropped from 203 knots to 160 knots while they were climbing to forty one thousand feet from thirty seven thousand feet. The airspeed of 160 is very close to the stalling speed of the aircraft. Around this stalling speed the captain told the first officer that they will be coming down in a second and then stated that this thing will not hold altitude. The engines started to spool down and the stick shaker and stick pusher were activated three times in a ten second interval. The airplane then entered an aerodynamic stall. Once the stall occurred a left rolling motion began which eventually ended up in an eighty two degree left wing down configuration. At this time both of the engines actually flamed out. The captain declared an emergency at this time but only admitted to one of the engines failing, not both of them.…

My product will be a class about the Army Leadership Requirement Model which will be focused on newly promoted Army Noncommissioned Officers and junior soldiers who are ready to work as a leader. Army Leadership Requirement Model consists of two major areas that all Army leaders must possess: Attributes “what a leader is” and the bottom portion consisting of Competencies “what a leader does.” It provides a common basis for thinking, learning, and applying leadership principles for all Army leaders. All of the model’s components interrelate with each other. The article “Mastering the Art of Military Leadership” relates to the subject because it indicates that a successful leader transformation from expert to master is not coincidence (Buckner, 2014). It is a thoughtful sense of purpose that is educated through years of professional and personal development.…
